Admission Criteria

Patients, who meet the following criteria and live in the 32 county service area, are eligible for admission:

  • have approximately six months or less to live if the disease progresses as expected
  • are no longer undergoing curative therapies
  • have a physician who will work with the Hospice program.
A family member, physician, member of a long-term care staff or hospital discharge planner can make referrals.

In most cases a team member can respond within 24 hours of referral to begin the admission process.
